What’s the difference between a panic attack, panic disorder, and general anxiety?

A panic attack is a sudden, intense surge of fear that peaks within minutes, often with physical symptoms like a racing heart, shortness of breath, dizziness, and a feeling of impending doom. Anyone can have one under extreme stress. Panic disorder is diagnosed when panic attacks recur unexpectedly and you start living in fear of the next one — changing your routine, avoiding places, or staying constantly on alert.

General anxiety tends to be a steadier, lower-grade worry about everyday events, whereas panic is acute and explosive. The two overlap and often coexist, which is why a proper assessment matters. A free 20-minute consultation is a low-pressure way to sort out what you’re actually dealing with.

Is a panic attack dangerous? It feels like a heart attack.

Panic attacks feel terrifying but are not physically dangerous — they will not stop your heart, make you faint, or cause you to lose control, even though every signal in your body says otherwise. What you’re feeling is your fight-or-flight system firing at full strength when there’s no real threat: the racing heart, chest tightness, and breathlessness are adrenaline doing exactly what it’s designed to do, just at the wrong moment.

That said, chest pain and a racing heart should be medically checked at least once, because the symptoms can mimic cardiac and other conditions. If you’re experiencing chest pain for the first time or are unsure whether it’s a panic attack or a medical emergency, treat it as an emergency and seek immediate medical care. Once a doctor has ruled out a physical cause, therapy can help you stop fearing the sensations themselves.

Why do panic attacks seem to happen “out of nowhere”?

Unexpected panic attacks are a hallmark of panic disorder, and the “out of nowhere” quality is part of what makes them so distressing. In reality, they’re usually triggered by small internal sensations — a slight change in heart rate, a deep breath, caffeine, fatigue, or a shift in body temperature — that your brain misreads as danger and amplifies into a full alarm response.

This is called fear of fear: you become hypersensitive to normal bodily cues and start scanning for them, which makes an attack more likely. It’s a self-reinforcing loop, not a character flaw or a sign something is medically wrong. The good news is that this exact loop is what evidence-based therapy is designed to interrupt.

What is agoraphobia, and how is it connected to panic disorder?

Agoraphobia is the fear of situations where escape might be difficult or where help wouldn’t be available if a panic attack happened — crowds, public transit, lineups, driving, or simply being far from home. It frequently develops after panic disorder, as avoidance becomes the brain’s strategy for preventing the next attack.

The problem is that avoidance shrinks your world and actually strengthens the fear over time. Treatment addresses both the panic and the avoidance together, gradually rebuilding your confidence to go where you want to go. The most powerful component is interoceptive exposure — deliberately and safely bringing on the physical sensations of panic so your brain learns they aren’t dangerous. What is interoceptive exposure, and is it as scary as it sounds?

Interoceptive exposure means safely and gradually recreating the bodily sensations that trigger your panic — for example, breathing through a straw to mimic breathlessness, spinning to create dizziness, or brief physical exertion to raise your heart rate. The point is to teach your nervous system, through direct experience, that these sensations are uncomfortable but harmless.

It sounds counterintuitive, but it’s done collaboratively, starting small, and always within your control — never sprung on you. Over repeated practice, the sensations stop setting off the alarm, and the “fear of fear” loop weakens. This is one of the reasons CBT for panic is often relatively fast-acting compared with treatment for other anxiety conditions.

Can panic disorder be cured, and how long does treatment take?

Panic disorder is one of the most treatable mental health conditions. With CBT, many people experience significant improvement within a relatively short course of therapy — often a few months of regular sessions — and a large share reach a point where panic attacks are rare and no longer control their decisions.

“Cured” is the wrong frame: the realistic goal is that an occasional surge of adrenaline no longer frightens you or dictates where you’ll go. The skills you build are durable and stay with you, so even if a stressful period brings symptoms back, you’ll know exactly how to respond. Your therapist tracks progress and adjusts the plan as you go.

Do I need medication for panic disorder, or is therapy enough?

Many people resolve panic disorder with CBT-based therapy alone. For more severe or long-standing panic, research supports combining therapy with medication — typically SSRIs — which can lower baseline anxiety enough to make the therapy work more accessible early on.
